Job summary

Health at Home HHA-AUSTIN, LLC in Austin is seeking a dedicated Registered Nurse to provide skilled nursing care in patients' homes. You will assess needs, complete OASIS, and coordinate with physicians and the interdisciplinary team to achieve patient goals.

Qualifications

• Current license as a Registered Nurse in good standing for the state.
• BSN degree preferred but not required.
• Bilingual and able to read, write and speak Spanish preferred.

Responsibilities

• Assess home health patients and identify physical, psychosocial and environmental needs.
• Complete OASIS, assessment and visit documentation per policy.
• Communicate significant findings to Home Health Director and physician; document interventions.
• Participate in development and implementation of the Plan of Care with the interdisciplinary team.
• Supervise home health aides and LPNs/LVNs to ensure quality and continuity of services.
