Job summary

Health at Home HHA-AUSTIN, LLC in Austin, TX is seeking a qualified Registered Nurse to provide coordinated skilled nursing care in patients' homes on an intermittent basis. You will collaborate with an interdisciplinary team to achieve patient goals and ensure high-quality care under physician orders and the Home Health Director.

The role involves assessing patients, completing OASIS, coordinating care, and supervising aides.

Qualifications

• Graduate of an accredited school of nursing.
• Current license as a Registered Nurse in Texas in good standing.
• BSN preferred; one year RN clinical experience required; acute/home care preferred.
• Maintains BCLS certification and valid driver’s license.

Responsibilities

• Assess home health patients identifying physical, psychosocial and environmental needs.
• Complete OASIS, assessment and visit paperwork per agency policy; document care coordination.
• Communicate significant findings to Home Health Director and physician; document actions.
• Identify and report problems; respond to life-threatening situations; coordinate care with other disciplines.
• Develop and implement Plan of Care (POC) with physician, patient, family and team.
• Supervise and direct home health aides and LPNs/LVNs to ensure quality care.
• Participate in interdisciplinary care conferences and quality improvement processes.
• Receive referrals and coordinate timely care with agency staff and providers.
• Ensure continuity of patient care; follow infection control and universal precautions.
