Be Healthy at Home, Inc. in Austin, TX is hiring an experienced Registered Nurse (RN) for home health services. You will assess patients, admit and discharge using OASIS, and collaborate with LVNs and HHAs to ensure high-quality care on the road.

We seek an active RN license, at least 1 year of home health experience, and a willingness to travel within the South Austin area. Full-time and PRN positions offer competitive pay, mileage reimbursement, and comprehensive benefits.

Qualifications

• Active RN license in good standing is required.
• Minimum of 1 year of home health nursing experience.
• Experience with case management and interdisciplinary collaboration.

Responsibilities

• Utilize OASIS for patient admissions, transfers, re-certifications, and discharges according to physician orders.
• Manage cases and collaborate with LVNs and HHAs to ensure high-quality patient care.
• Maintain full compliance with all agency policies and procedures.
