Qualifications

• Graduate from an accredited nursing program; RN license required in state of work.
• BSN preferred; BLS required per policy guidelines.
• Experience in home health nursing and care coordination preferred.

Responsibilities

• Manage an assigned patient caseload in the home setting with timely care and documentation.
• Collect data from assessments, telephonic contacts, and EMR; analyze to plan care.
• Coordinate multi-disciplinary care and supervise LPNs/HHAs to ensure quality outcomes.
