Job summary

Advocate Health Care is seeking a Registered Nurse (RN) Case Manager for Home Health in Illinois. The nurse will manage a patient caseload, coordinate care plans, and work closely with LPNs and an interdisciplinary team to deliver high-quality, patient-centered home health care.

This full-time role requires an active IL RN license, CPR certification, a valid driver's license, and at least one year of medical-surgical nursing experience.

Qualifications

• Current Illinois Registered Nurse (RN) license.
• Graduate of an accredited school of nursing.
• Minimum one year of medical-surgical nursing experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong communication, critical thinking, and organizational skills.
• Basic computer and documentation proficiency.

Responsibilities

• Perform skilled nursing visits in patients' homes.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Monitor patient progress and adjust care plans as needed.
• Coordinate care with LPNs and interdisciplinary team members.
• Educate patients and families regarding disease management, medications, treatments, and health maintenance.
• Identify changes in patient conditions and implement appropriate interventions.
• Coordinate referrals, discharge planning, and community resources.
• Maintain accurate and timely documentation per regulatory and organizational requirements.
• Promote patient safety, quality outcomes, and evidence-based nursing practice.
• Participate in quality improvement initiatives and team collaboration activities.
