Job summary

Advocate Health in Macon, GA is seeking an RN Home Health to manage an assigned patient caseload in the home, coordinate care with a multidisciplinary team, and deliver evidence-based nursing care per plan of care. The role requires a BSN preferred, current RN license, and BLS; schedule Monday-Friday with occasional weeknight and weekend calls; 8:30-5 with on-call after hours; sign-on bonus up to $3,000.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Current RN license in the state of practice; BLS required.
• Experience in home health care and patient assessments.

Responsibilities

• Manage daily patient caseload in home setting with timely care and documentation.
• Collect data from assessments, telephonic contacts, and EMR; analyze for care planning.
• Coordinate multidisciplinary team and supervise LPNs/HHAs as needed.
• Perform treatments and procedures per policy and state regulations.
