Advocate Health Care in Chicago’s west side area is seeking a Registered Nurse – Home Health to provide in-home patient care, assess needs, and coordinate with the multidisciplinary team. This role emphasizes therapeutic nurse-patient relationships, adherence to evidence-based standards, and support for patient education and safety.

The position requires an Illinois RN license, 1 year of medical-surgical experience, and availability for on-call and weekend rotations.

Qualifications

• Graduate from an accredited school of nursing.
• Earn 10 continuing education credits annually.
• One year of medical surgical experience.
