Advocate Health is seeking a Registered Nurse – Home Health to support patients across Oswego, Montgomery, Aurora, Plainfield and adjacent Illinois communities. The role requires delivering in-home nursing care Monday through Friday, with weekend/on-call rotation and holiday coverage as needed.

Candidates must hold an Illinois RN license, complete continuing education annually, and demonstrate strong communication, documentation and clinical skills for high-quality patient outcomes.

Qualificações

• Graduate from an accredited school of nursing.
• Maintain current Illinois RN license.
• Earn 10 continuing education credits annually.

Responsabilidades

• Develops a therapeutic nurse/patient relationship throughout the care continuum.
• Assess patient and family readiness; involve them in planning and care.
• Deliver individualized care to promote physical and psychosocial comfort.
• Coordinate care using interdisciplinary teamwork and evidence-based practice.
