Job summary

Qualifications

• Texas RN license in good standing.
• At least 2+ years of home health RN experience.
• Current CPR certification.
• Reliable transportation and valid Texas license.

Responsibilities

• Case management and supervised visits.
• Perform initial and ongoing patient assessments.
• Chart vitals and observations at each visit.
• Provide skilled nursing care per care plan.
• Educate patients on home health procedures.
• Maintain accurate visit notes within 24 hours.
• Protect patient PHI under HIPAA.
• Support palliative care as needed.
