Job summary

Frontpoint Health is seeking a dedicated Registered Nurse (RN) for a PRN/Per Visit role in Haslet, TX. The RN will be responsible for patient care management, including OASIS visits and skilled nursing tasks, ensuring continuity of care for patients in their homes.

Qualified candidates must possess a current Texas State RN license and have at least 2 years of experience in home health care. This position requires reliable transportation and adherence to patient confidentiality through HIPAA compliance.

Qualifications

• 2+ years of experience as a Registered Nurse in Home Health.
• Reliable transportation is required.
• Texas Driver's License and Auto Liability Insurance are necessary.
• Current TB Skin Test must be provided.

Responsibilities

• Manage case and supervise visits.
• Perform skilled and comprehensive patient assessments.
• Chart observations of patient's condition during visits.
• Provide skilled nursing care per the plan of care.
• Educate patients on home health procedures.
• Maintain clinical records of each visit in a timely manner.
• Ensure confidentiality of patient information.
