Responsibilities

• Case management and supervised visits.
• Initial patient assessment during admission.
• Assess and chart observations at each visit (vital signs).
• Provide skilled nursing care per the plan (medication administration, wound care).
• Educate patients on home health procedures and strategies.
• Maintain clinical records (visit notes within 24 hrs, OASIS within 48 hrs, medication profile).
• Maintain patient confidentiality under HIPAA; support comfort with palliative care.
