The ideal candidate holds a Texas RN license, has 2+ years of home health experience, CPR certification, reliable transportation, and a valid TX driver’s license with auto liability insurance. Must maintain accurate charting and patient education.

Qualifications

• Current Texas State license as a Registered Nurse.
• 2+ years of RN experience in Home Health.
• Current CPR certification.
• Reliable transportation and Texas driver’s license with auto liability insurance.

Responsibilities

• Case management and supervised visits.
• Initial admission skilled nursing assessment.
• Assess and chart patient condition at each visit (vital signs).
• Provide skilled nursing care per plan of care (medication, wound care).
• Provide patient education for home health care procedures.
• Maintain clinical records: visit notes within 24 hours; OASIS within 48 hours; medication profile, orders/plan.
• Maintain patient PHI confidentiality per HIPPA.
• Keep the patient comfortable through palliative care.
