Job summary

Frontpoint Health in Balch Springs, TX is seeking a Registered Nurse with Home Health experience to provide skilled nursing visits in patients’ homes, including OASIS visits and supervised visits, coordinating with the interdisciplinary team.

You will administer care per the plan of care, document visit notes within 24 hours and 48 hours for OASIS, and educate patients and families while maintaining HIPAA compliance.

Qualifications

• Current Texas State license as a Registered Nurse.
• 2+ years experience as an RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Texas Driver's License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience (Kinsser preferred).

Responsibilities

• Case management and supervised visits.
• Perform skilled assessment during initial admission.
• Assess and chart observations at each visit (vital signs).
• Provide skilled nursing care per plan of care (medication administration, wound care).
• Educate patients on home health procedures and strategies.
• Maintain clinical records: visit notes within 24 hours, OASIS within 48 hours, medication profiles, orders/plan submitted timely.
• Maintain patient confidentiality under HIPAA.
• Support patient comfort through palliative care.
