You will assess, plan, and document care, collaborate with physicians and discharge planners, and participate in ongoing professional development to ensure high-quality outcomes for patients and families in their homes.

Qualifications

• Active RN license required.
• Minimum 1 year of clinical experience.
• Strong communication and organizational skills.
• Ability to work independently.
• Valid driver's license and reliable transportation.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Plans, coordinates, and ensures quality home health care.
• Delivers nursing services in the home and provides case management.
• Monitors changing client and family needs and outcomes.
• Performs initial assessments and tracks progress toward goals.
• Maintains ongoing communication with patient families, physicians, and staff.
• Completes clinical documentation (OASIS, evaluations, orders).
• Collaborates with physicians and discharge planners; promotes health education.
• Interacts with community health resources and supports ongoing education.
