Job summary

Qualifications

• Current Texas RN license and CPR certification.
• Minimum 2 years of Home Health RN experience.
• Reliable transportation with valid Texas driver's license and auto insurance.
• TB skin test current or completed.
• Experience using EMR software (Kinnser preferred).

Responsibilities

• Case management and supervised visits.
• Perform skilled assessment during admission and visits.
• Chart patient observations at each visit (vital signs).
• Provide skilled nursing care per care plan (medications, wound care).
• Educate patients and families on home health procedures.
• Maintain complete visit notes within 24 hours and OASIS within 48 hours.
• Maintain patient PHI confidentiality under HIPAA.
• Support patient comfort through palliative care.
