Job summary

Qualifications

• Current Texas State license as a Registered Nurse.
• 2+ years experience as RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Texas Driver's License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience (Kinnser preferred).

Responsibilities

• Case management and supervised visits.
• Perform skilled assessment of the patient during initial admission.
• Assess and chart observations of patient’s condition at each visit.
• Provide skilled nursing care per plan of care (medication, wound care).
• Provide patient education for home health care procedures.
• Maintain clinical records (visit notes, OASIS where applicable).
• Maintain confidentiality of patient PHI under HIPAA.
• Provide palliative care to keep patient comfortable.
