Job summary

Frontpoint Health is seeking a full-time Registered Nurse (RN) for home health care to manage and coordinate patient care in their place of residence. Candidates must hold a current Texas State license as an RN and possess at least 2 years of home health experience.

This role includes performing comprehensive patient assessments, providing skilled nursing care, and ensuring confidentiality under HIPAA laws. Reliable transportation and CPR certification are also required. The RN will work closely with an interdisciplinary team to ensure high-quality care for patients.

Qualifications

• Registered Nurse must have a current Texas State license.
• Minimum of 2 years experience in Home Health required.
• CPR certification must be current.

Responsibilities

• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit.
• Provide skilled nursing care in accordance with the plan of care.

Skills

Current Texas State license as a Registered Nurse

2+ years experience as an RN in Home Health

Current CPR certification

Reliable transportation

Texas Driver’s License and Auto Liability Insurance

Current TB Skin Test

EMR experience (Kinsser preferred)
