Job summary

Frontpoint Health is seeking a Registered Nurse (RN) for Home Health in Grapevine, TX. The RN will administer skilled nursing care in patients' homes, coordinate with the interdisciplinary team, and manage visits within license scope including OASIS and supervised visits.

Role involves case management and timely documentation. The ideal candidate has a Texas RN license, 2+ years Home Health experience, CPR certification, and reliable transportation to service assigned visits.

Qualifications

Texas RN license required.
2+ years Home Health RN experience.
Current CPR certification.
Reliable transportation required.
Texas Driver's License and auto liability insurance.
Current TB skin test.
EMR experience (Kinsser preferred).

Responsibilities

Case management and supervised visits.
Perform skilled and comprehensive assessment of the patient during the initial admission.
Assess and chart observations of the patient's condition at each visit (vital signs).
Provide skilled nursing care as needed per the plan of care (administer medications, wound care).
Provide patient education for home health care procedures and strategies.
Maintain clinical records: visit notes within 24 hours, OASIS within 48 hours, etc.
Maintain patient confidentiality per HIPAA.
