Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and health care team.
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks per regulations.
• Performs supervisory visits and ensures high-quality clinical services.
• Completes comprehensive assessments (OASIS) and medication reconciliation accurately.
• Documents patient visits per policy and payer requirements.
