Job summary

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provides and directs nursing care to patients in their homes as prescribed by the physician and in alignment with agency policies.
• Coordinates total plan of care with other healthcare professionals and communicates with physicians, family, and community resources.
• Performs supervisory visits, documents care delivery, and ensures quality of care and patient safety.
