Job summary

Advocate Health is hiring a Registered Nurse for Home Health to cover the City of Chicago’s west side communities, including Logan Square. The role is full‑time, Monday–Friday, 8a–5p with rotating weekends and on‑call shifts.

A sign‑on bonus of up to $3,000 is available for qualified candidates. Responsibilities include building strong nurse–patient relationships, delivering evidence‑based care, educating patients and families, applying the nursing process, and collaborating with the care team

Qualifications

• Graduate from an accredited school of nursing.
• Current Illinois RN license.
• CPR certification and continuing education.
• Strong communication and teaching skills.
• Experience in home health or medical–surgical setting preferred.

Responsibilities

• Develops and maintains therapeutic nurse–patient relationships.
• Delivers high‑quality, cost‑effective home healthcare aligned with standards.
• Educates patients and families to promote understanding and outcomes.
• Uses the nursing process to assess, plan, implement, and evaluate care.
• Collaborates with the care team and participates in professional development.
