Job summary

Advocate Health is seeking a Registered Nurse for home health in the Illinois service area. The role requires building therapeutic nurse-patient relationships, delivering holistic care, and coordinating with an interdisciplinary team.

Schedule is Monday–Friday 8a–5p with on-call weekends and holidays; sign-on bonus up to $3,000 for eligible internal candidates. Travel to covered communities within Advocate service area may be required.

Qualifications

• Graduate from an accredited school of nursing.
• Earn 10 continuing education credits annually.
• One year of medical surgical experience.
• Has demonstrated proficiency for the RN role.

Responsibilities

• Nurse/patient relationship: develops therapeutic relationship and plans care.
• Assuring/improving quality care: ensures access to quality care and updates practice per standards.
• Patient/staff education: supports learning for patients and staff.
• Nursing process: uses nursing process for care planning and decision making.
