Job summary

CHI Franciscan Health at Home is seeking a Registered Nurse to deliver compassionate, patient‑centered care in the home setting. You will perform thorough assessments, implement and adjust care plans, and collaborate with an interdisciplinary team to improve outcomes.

This role requires at least 1 year of nursing experience, an active WA RN license, CPR/BLS, and a valid driver’s license. Two years of homecare/hospice experience is preferred. No after-hours on-call.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in the state served (Washington).
• Minimum of 1 year of nursing experience.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Two years of hospital/hospice/homecare RN experience preferred.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient’s environment.
• Craft, implement, and adjust comprehensive care plans to promote independence and well‑being.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing crucial teaching and emotional support.
• Mentor and guide LPNs/LVNs and Home Health Aides to foster a cohesive team.
• Utilize AI-powered clinical documentation and best practices to optimize care delivery and documentation.

Required

• Graduate of an accredited school of nursing.
• Must have an active RN license in the state served.

• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Preferred

• Two years of hospital / hospice / homecare RN nursing experience preferred.
