Qualifikationen

• Active RN license in the state of practice and at least 1 year of clinical nursing experience.
• CPR certification or ability to obtain it.
• Current driver's license with reliable transportation.

Aufgaben

• Provide one-on-one skilled nursing in patients' homes to promote recovery and independence.
• Conduct in-home assessments evaluating physical, emotional, and environmental needs.
• Perform wound care, infusions, catheter care, med management, and post-operative care as appropriate.
• Educate patients and caregivers to manage symptoms, meds, and daily home care.
• Monitor progress, identify changes, and coordinate with physicians and care teams.
