Job summary

Aveanna Healthcare is seeking a Registered Nurse (Home Health) for full-time work across Dane County, including Waunakee, Middleton, and Northeast Madison. You will deliver one‑on‑one nursing care in patients' homes, conduct in‑home assessments, and develop patient-centered care plans to support recovery and independence.

Responsibilities include skilled nursing interventions, caregiver education, and close collaboration with the interdisciplinary team to prevent hospitalizations.

Qualifications

• Must hold an active Registered Nurse (RN) license in the state of practice
• 1+ year of clinical nursing experience preferred
• CPR certification or ability to obtain CPR certification

Responsibilities

• Serve as the primary clinical partner for home health patients and families, delivering one-on-one nursing care
• Conduct in-home assessments to evaluate physical, emotional, and environmental needs and translate findings into patient-centered care plans
• Provide skilled nursing interventions including wound care, IV infusions, catheter care, medication management, and post‑operative care
• Educate patients and caregivers to manage symptoms and daily care at home
• Monitor patient progress, identify changes in condition early, and communicate with the medical team to adjust plans
• Coordinate care across providers to ensure continuity and high-quality outcomes
• Complete timely documentation reflecting the clinical picture and supporting quality and compliance
