Qualifications

• Active WI RN license with 1+ year clinical nursing experience.
• Patient-centered, organized, and compassionate.
• Valid driver's license and reliable transportation.
• CPR certification or ability to obtain.
• Part-time availability 3+ days/week.

Responsibilities

• Provide in-home, one-on-one nursing care for patients in their homes.
• Perform comprehensive in-home assessments to guide care plans.
• Deliver skilled nursing interventions (wound care, infusions, catheter care, med management).
• Educate patients and caregivers to manage symptoms and daily care at home.
• Monitor patient progress and communicate with physicians and care team.
• Coordinate care across providers and document accurately for quality outcomes.
