Aveanna Healthcare in Wisconsin seeks a Registered Nurse (Home Health) to provide one-on-one in-home nursing care, promoting recovery, safety, and independence. Travel within the service area is required, including Dodge, Washington, Ozaukee, Milwaukee, and Waukesha.

The role emphasizes patient-centered care, wound care, medications management, caregiver coaching, and collaboration with the care team. Full-time position with a Sunday-Saturday 7-day rotation.

Qualifications

• Active Registered Nurse (RN) license in the state of practice.
• At least 1 year of clinical nursing experience.
• Driver's license, reliable transportation, and auto insurance.
• CPR certification (or ability to obtain)

Responsibilities

• Provide one-on-one, in-home nursing care to promote recovery, safety, and independence.
• Perform comprehensive in-home assessments and create patient-centered care plans.
• Deliver skilled nursing interventions: wound care, infusions, catheter care, meds, post-op and chronic disease management.
• Educate and empower patients and caregivers on symptom management and daily care at home.
• Monitor progress and communicate with physicians and care team to adjust plans.
• Coordinate care across providers to ensure continuity and high-quality outcomes.
• Complete timely, accurate documentation reflecting the clinical picture.
