Qualifications

• RN license issued by the state where you practice is required.
• BLS for Healthcare Providers certification within 6 months unless exempted by leadership.

Responsabilités

• Assumes responsibility for caseloads in specialty areas and conducts initial assessments.
• Establishes care plans in line with physician orders and patient needs.
• Provides direct patient care and updates care plans as patient condition changes.
• Completes documentation including visit assessments, orders, and instructions accurately and promptly.
• Communicates with physicians and care team to update and coordinate patient plans.
• Facilitates discharge planning and ensures continuity of care after discharge.
