Job summary

Advocate Health is seeking an experienced registered nurse to manage diverse caseloads in specialty areas. You will provide direct patient care, assess needs, and modify care plans in collaboration with physicians and families.

The role requires a state RN license, an Associate's Degree in Nursing, and typically 3 years of clinical nursing experience. You will document care, support discharge planning, and participate in quality improvement activities within our care teams.

Qualifications

• RN license in the state where practice is located.
• Associate's Degree in Nursing required; BSN preferred.
• Minimum 3 years of clinical nursing experience.

Responsibilities

• Provide direct patient care and adjust nursing plans as needed.
• Conduct initial assessments and develop care plans per physician orders.
• Complete documentation including assessments and orders with accuracy.
• Communicate with physicians and care team to update patient plans.
• Participate in discharge planning and care continuity at home.
