• Communicate effectively with patients, families, and providers

Responsibilities

• Initiates and revises the plan of care with the physician and team
• Supervises aides and nurses; assigns tasks per regulations
• Provides required supervisory visits
• Delivers high-quality clinical services within plan of care and infection control standards
• Completes comprehensive assessments and medication reconciliation
• Documents visits per policy and payer requirements
