Job summary

Advocate Health is seeking a dedicated Registered Nurse to join our care team. You will perform scheduled visits, assess patients across physical, psychosocial, and environmental domains, and update plans of care with each visit.

You will collaborate with physicians and the RN Case Manager to adjust treatment plans, monitor responses, and ensure safe discharge planning with community resources. Strong communication and organization are essential to provide high-quality, compassionate care.

Qualifications

• Registered Nurse license in the state of practice (WI or IL).
• Basic Life Support (BLS) certification required upon hire.
• Valid driver license with access to a car and insurance.
• Typically 1 year of nursing experience.
• Proficiency in clinical skills and sound judgment.

Responsibilities

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Assesses patients' physical, psychosocial, and environmental needs and updates care plans.
• Communicates with physicians to confirm and update treatment plans; relays changes to the RN Case Manager.
• Monitors patient responses to treatments and revises care plans as needed.
• Participates in discharge planning and coordinates community resources for continuity of care.
• Documents and updates patient records per agency, state, and federal guidelines.
• Ensures adherence to policies and safety procedures; performs positioning and transporting duties as required.
