Job summary

Advocate Health seeks a licensed nurse to manage patient care during assigned visits and collaborate with the care team to update plans of care. You will perform comprehensive health assessments, document findings, and communicate treatment updates to physicians and the primary RN.

You will assist with discharge planning, coordinate scheduling, and ensure continuity of care after discharge while following all agency and state guidelines.

Qualifications

• Proficiency in clinical nursing tasks and patient assessment.
• Excellent written and verbal communication with patients and team.
• Ability to build rapport with patients, families, and physicians.
• Demonstrated independent decision making and prioritization.
• Strong organization and problem-solving abilities.
• Ability to multitask with frequent interruptions.
• Must have a valid driver's license and access to a car with insurance.

Responsibilities

• Provide nursing care and assess patient needs during assigned visits.
• Collaborate with interdisciplinary team to update plans of care.
• Document and update patient records per guidelines and policy.
• Communicate plan changes to physicians and the RN supervisor.
• Assist with discharge planning and continuity of care after discharge.
• Schedule visits and case conferences with the primary RN.
• Follow policies, safety procedures, and maintain compliance.
