Caretenders Home Health, part of the LHC Group and UnitedHealth Group family, seeks a Registered Nurse to provide and direct nursing care in patients' homes according to physician orders and policy. You will coordinate the plan of care with the care team and communicate with patients and families to ensure continuity and quality of care.

Competenze

• Current and unrestricted RN licensure in state of practice.
• Valid driver's license and access to dependable vehicle or public transportation.
• Current CPR certification or ability to obtain within 90 days of hire.
• 1+ year of Home Health experience.
• Ability to work independently.
• Strong communication, writing, and organizational skills.

Mansioni

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Performs comprehensive nursing assessments and ensures accurate documentation.
