Job summary

Caretenders, part of LHC Group, a leading post-acute care partner for hospitals, physicians and families nationwide, seeks a Registered Nurse in Home Health to provide nursing care in patients’ homes and coordinate care with physicians and families.

You will develop and revise care plans, supervise home health staff, and ensure compliance with laws and agency policies; CPR certification and home health experience are preferred.

Qualifications

• Current and unrestricted RN licensure in state of practice
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR Certification (preferred) or ability to obtain within 90 days of hire
• 1+ years of Home Health experience (preferred)
• Ability to work independently
• Solid communication, writing, and organizational skills

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician
• Supervises care provided by home health aides and licensed nurses
• Provides supervisory visits as required
• Performs comprehensive assessments (OASIS) and medication reconciliation
• Documents patient visits per policy and payer requirements
• Communicates with physicians, patients, and families to ensure quality care
• Participates in survey readiness and performance improvement activities
• On-call and weekend rotation as needed
