Job summary

Caretenders Home Health, a part of LHC Group and UnitedHealth Group, is seeking a Registered Nurse to join a national post-acute care team dedicated to caring for patients in their homes. You will initiate and revise the plan of care with physicians, supervise home health aides and licensed nurses, and perform supervisory visits while ensuring quality and infection control.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License with access to a dependable vehicle or reliable transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Makes nursing evaluations, determines eligibility and medical necessity, and coordinates with physicians.
• Communicates effectively with agency staff, physicians, patients, and families to ensure quality care.
• Participates in on-call and weekend rotation as needed and supports performance improvement initiatives.
