Caretenders, a part of LHC Group, is seeking a Registered Nurse in Home Health to provide and direct nursing care in patients' homes, coordinating care with physicians and the care team to ensure continuity of care.

Competenze

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Mansioni

• Provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations, and agency policies.
• Coordinates total plan of care with other health care professionals to ensure continuity of patient care.
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks per regulations.
• Documents visits and completes comprehensive assessments (OASIS) accurately and timely.
• Communicates with physicians, patients, and family to ensure quality care and satisfaction.
• Participates in on-call and weekend rotations and supports survey readiness and performance improvement.
