Job summary

At Home Healthcare, part of the LHC Group within the Optum family, is seeking a Registered Nurse in Home Health to deliver and coordinate nursing care for patients in their homes under physician orders. The role emphasizes plan of care development, team supervision, and thorough documentation to ensure quality outcomes.

Your responsibilities include conducting comprehensive assessments, coordinating with physicians and the care team, and participating in on-call rotations as needed.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR Certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience; ability to work independently; strong communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks per regulations.
• Provides supervisory visits and ensures quality, documentation, and coordination with care team.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Coordinates with physicians, patients, families, and community resources to ensure continuity of care.
