Job summary

Elara Caring is seeking a Home Health Baylor Registered Nurse to join our Worcester team. This full-time role offers one-on-one patient care in home settings, with autonomy and strong clinical support to drive positive outcomes.

You will assess, plan, and deliver skilled nursing services, educate patients and caregivers, and coordinate with physicians and the interdisciplinary team. Travel throughout the assigned territory is required, with a guaranteed salary plus productivity bonus based on

Qualifications

• Current, unrestricted RN license.
• Associate Degree in Nursing required; BSN preferred.
• At least one year of clinical nursing experience.
• Home health experience is required.
• Homecare Homebase experience preferred.

Responsibilities

• Assess patients and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders.
• Monitor patient conditions, medications, and treatment progress.
• Educate patients and caregivers on treatments and disease management.
• Coordinate care with physicians and the interdisciplinary team.
• Maintain accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations and emergency room visits.
