Zusammenfassung

Elara Caring is seeking a Home Health Registered Nurse to provide skilled, personalized care in patients' homes across the Newburyport territory. You will manage a dedicated caseload, coordinate with an interdisciplinary team, and deliver care that helps patients recover, stay safe, and maintain independence.

This role offers autonomy, a supportive team, and a competitive compensation package with benefits and a sign-on bonus up to $5,000.

Qualifikationen

• A degree or diploma from an accredited nursing program.
• A current, unrestricted Registered Nurse license in the applicable state.
• At least one year of nursing experience in a clinical care setting.
• Valid driver's license and ability to travel throughout the territory.

Aufgaben

• Complete initial and ongoing nursing assessments in patients’ homes
• Manage assigned caseload and coordinate care
• Develop, implement, and update individualized Plans of Care
• Provide skilled nursing treatments in accordance with physician orders
• Monitor patients’ conditions, medications, symptoms, and responses to treatment
• Coordinate services with physicians, therapists, social workers, aides, and other care partners
• Maintain accurate, timely, and complete clinical documentation
• Follow all clinical, safety, regulatory, and HIPAA requirements
