Job summary

Elara Caring seeks a Behavioral Health Registered Nurse to provide in-home behavioral health care in the Worcester area. You will manage an assigned caseload, collaborate with a clinical team, and support patients to stay safe and independent at home.

Eligibility includes an MA RN license and at least 1 year of nursing experience; travel is approximately 50% within the Worcester coverage area. Generous benefits and education opportunities are offered.

Qualifications

• Graduate of an accredited nursing program (RN).
• Current, unrestricted MA RN license.
• At least 1 year nursing experience in a clinical setting.
• Behavioral health/home health experience preferred.

Responsibilities

• Manage assigned caseload for behavioral health in home visits.
• Develop and evaluate individualized plans of care.
• Perform initial evaluations and ongoing reassessments.
• Provide skilled behavioral health nursing visits per plan of care.
• Educate patients and families on conditions, medications, and resources.
• Document care accurately and timely.
