Job summary

Elara Caring in Worcester is hiring a Home Health Baylor Registered Nurse to provide skilled nursing care in patients' homes with a guaranteed salary and a $5,000 sign-on bonus. The role emphasizes one-on-one patient care, a 36-hour schedule paid as 40, and a flexible, independent field rhythm.

You will assess patients, develop care plans, monitor progress, educate families, and coordinate with the interdisciplinary team while maintaining timely documentation.

Qualifications

• Current, unrestricted RN license.
• Associate Degree in Nursing required; BSN preferred.
• At least one year of clinical nursing experience.
• Home health experience is required.
• Homecare Homebase experience preferred.

Responsibilities

• Assess patients and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders.
• Monitor patient conditions, medications, and treatment progress.
• Educate patients and caregivers on treatments and disease management.
• Coordinate care with physicians and the interdisciplinary team.
• Maintain accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations and emergency room visits.

• Valid driver's license, dependable transportation, and auto insurance
• Ability to travel throughout the assigned territory
• Ability to sit, stand, bend, lift and move intermittently and lift 50-100 lbs.
