Windsor Home Health is seeking an experienced Registered Nurse (RN) to lead and coordinate home-based patient care. You will perform assessments (including OASIS) and craft care plans that fit each patient, while supervising LPNs and Home Health Aides during visits.

Qualificações

• RN license in Ohio is required.
• Home health experience preferred.
• Strong organization and time management skills.

Responsabilidades

• Lead and coordinate patient care in collaboration with physicians.
• Perform assessments (including OASIS) and develop fit-for-patient care plans.
• Supervise and support LPNs and Home Health Aides during visits.
• Communicate closely with families and interdisciplinary team members.
