Job summary

Enhabit Home Health & Hospice is seeking a Registered Nurse (RN) to provide patient visits and coordinate care in the home setting. The role involves consulting with physicians, ensuring accurate OASIS collection, updating medication profiles, and timely reporting of lab values.

The RN will also participate in weekly case conferences and monthly case manager meetings, supporting high-quality patient outcomes and compliant care delivery.

Qualifications

• Be currently licensed as a Registered Nurse (RN) in the state of employment.
• A minimum of one year of clinical experience is preferred.
• Demonstrate knowledge and skill in current nursing practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Perform patient visits and coordinate client care.
• Oversee visit frequency for the episode and liaise with physicians and office staff.
• Provide updates on patient care, including medication profiles and lab values.
• Collect OASIS data accurately and participate in weekly case conferences and monthly case manager meetings.
