Job summary

Well Care Health is seeking a Home Health Registered Nurse to provide compassionate, patient-centered care in patients’ homes. The RN assesses, plans, and evaluates health needs, coordinating with a multidisciplinary team to ensure optimal outcomes for diverse age groups.

The role requires an active nursing license in the state of care, CPR certification, and 1+ year of RN experience with preference for home health experience. Flexible schedule and supportive environment offered.

Qualifications

• Graduate of an accredited school of nursing (AD, Diploma, or BSN).
• Current license to practice nursing in the state where care is provided (NC/SC).
• Minimum 1 year RN experience; 2+ years total clinical experience; home health experience preferred.
• CPR certification required.

Responsibilities

• Assess and monitor patients in their home environment.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
• Collaborate with a multidisciplinary team to ensure optimal outcomes.
• Manage a caseload and coordinate care with the team.
• Maintain documentation and ensure compliance with agency policies.
