Job summary

Specialized Home Care is seeking compassionate Registered Nurses (RNs) to work one-on-one with patients in their home environment to establish and oversee all aspects of case-management. You will create a Plan of Care, follow physician orders, and drive outcomes of increased patient independence and satisfaction.

Qualifications

• Minimum of 2 years of nursing experience (preference for acute/post-acute settings).
• Active State Registered Nurse licensure is required.
• Proof of CPR certification and valid driver's license.

Responsibilities

• Assumes responsibility for assigned patient’s care from admission through discharge by executing the Plan of Care.
• Collaborates with patient, family/caregiver(s), physicians, and care team members.
• Submit accurate, complete, timely documentation, including progress and clinical notes.
• Participate in case conferences reviewing services and goals with clinicians.
• Participate in on-call rotation as assigned.
