Qualifications

• RN license in good standing with the State Board of Nursing.
• CPR certification and valid driver’s license.
• Prefer at least one year nursing experience (home health, clinical, surgical or critical care).
• Self-directed with the ability to work with little supervision; organized.
• Excellent observation and communication skills; basic math and nursing skills.

Responsibilities

• Complete an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluate patient nursing needs and revise plans as needed.
• Develop care plans with goals based on nursing diagnoses and family involvement.
• Administer medications and treatments as prescribed and provide health education.
• Counsel patient and family in meeting nursing and related needs.
• Prepare daily clinical notes and update the physician every sixty days or as required.
• Participate in on-call duties and ensure equipment and services are available.
• Instruct, supervise and evaluate home health aide care every fourteen days.
