Job summary

Capital Health Care Network seeks a Registered Nurse to plan, organize and direct home health services for patients in their homes across Ohio. The role emphasizes community health education, independence, and compassionate care.

You will work with an interdisciplinary team, conduct initial assessments, develop and revise care plans, provide medications and instructions, and participate in on-call coverage to ensure high-quality patient outcomes.

Qualifications

• Must be licensed as an RN through the State Board of Nursing.
• CPR certification and a valid driver's license are required.
• At least 1 year of general nursing experience preferred.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Regularly re-evaluates patient nursing needs and revises the care plan.
• Develops and implements a comprehensive care plan with patient/family involvement.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family on nursing and related needs and provides education.
