Job summary

Alternate Solutions Health Network is seeking a Registered Nurse to provide high-quality home-based nursing care across assigned Dublin, Lewis Center, and surrounding areas in Ohio. You’ll work one-on-one with patients in their homes, delivering care directed by physicians and coordinating with an interdisciplinary team.

You will manage schedules with a flexible pattern, travel within the territory, and leverage your leadership skills to ensure optimal outcomes and patient satisfaction.

Qualifications

• Active RN license in Ohio
• Minimum one-year RN experience in acute care
• Home care experience preferred
• Valid driver's license and auto insurance in your name as a driver
• Capable of all physical demands

Responsibilities

• Provide skilled nursing care in a patient's home as directed by the attending physician
• Work autonomously and manage time efficiently
• Coordinate care with an interdisciplinary team and communicate with families
• Document care plans and follow agency protocols and Medicare guidelines
• Assist with admissions, transfers, and discharges while ensuring quality care
