Adoration Health in Providence, RI is seeking a dedicated Home Health Registered Nurse to deliver patient-centered care in a 1:1 home setting. You will follow MD orders, develop tailored care plans, and work with an interdisciplinary team to meet patient goals.

The role offers full-time hours with flexible scheduling, driving required, and opportunities for growth within a supportive environment committed to compassionate home health delivery.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) required in state of practice.
• Minimum of one year nursing practice; home health or geriatrics preferred.
• Valid driver's license and proof of car insurance per policy.
• New grads may be considered in select markets.
• Current CPR certification required.

Responsabilités

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and goals.
• Provide nursing care: medications, treatments, interventions.
• Pain and symptom management for patients.
• Educate and support family and caregivers.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required.
