Job summary

Adoration Health's Home Health and Hospice is seeking a dedicated Home Health RN to join our Providence, RI team, delivering compassionate 1:1 patient care in home settings. You will assess needs, create care plans aligned with physician orders, and coordinate with families to achieve patient goals.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) with current unrestricted license in the state of practice and residence.
• One year nursing practice in a patient care setting required; home health/geriatrics preferred.
• Valid driver's license, valid insurance per Adoration policy.
• New grads may be considered in select markets.
• Current CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
